  女児として播磨地域の小学校に通う性同一性障害GID)の男児(7つ)を診察している大阪医
科大学付属病院精神神経科大阪府高槻市)の康(こう)純 (じゅん)医師が十八日までに、神戸新聞社の取材に応じた。康医師は昨年二月、男児と初めて対面。以来、男児の長期休暇などに診察を続けているといい、 「社会は子どもが自分の思いを打ち明けたときに受け止めてあげるべき」と理解を呼びかけた。康医師との一問一答は次の通り。(霍見真一郎)
 -男児が六歳という非常に幼いとき、診断書を書いた
  「こんなに幼いケースは初めてだった。保育園の年中組から年長組にかけて、首尾一貫して自分は女だと主張しており、少なくとも二年以上にわたって性自認男児の心の性)が変わっていないことなどから、GIDと診断した。しかし子どものGIDは成人になって体の性に戻る可能性もあり、今後どう変わっていく かは別問題だ」
 -子どものGIDの問題点は
 「性自認は自我の大きな要素。親や学校に押さえ込まれて成長すると、自分が生きていくことの価値が見いだせなくなり、自傷行為や自殺などにつながる可能性がある」
 -学校の受け入れで、教員に必要なことは
 「きちんとした知識を持つこと。生徒が打ち明けてきたときには、まず受け入れてほしい。対応に迷ったなら、医師の意見を聞いてほしい」
 -教育行政に何を求めるか
 「世の中には、GID患者が必ず一定の割合でいる。おちんちんが付いている女の子がいてもいい、という雰囲気をつくっていくことが大切。GIDの子どもへの対応策を定めるべきだ。今後、専門家の意見を聞き、そういった子どもを受け止める体制づくりを進めてほしい」

兵庫県の小学2年男児(7つ)が性同一性障害と診断され、入学時から女児として通学していることが18日わかった。第2次性徴前の小学低学年児が性同一性障害と診断された例は極めて異例。専門家はおおむね肯定的だが、「長期的な問題」と指摘する声もある。
スカートをはいて通学し、いつも女の子たちと遊び、ぬいぐるみが大好きな小学2年の女児は、実は性同一性障害男児文科省は、小学校で同様事例は聞いたことがないという。
地元教育委員会によると、男児は5歳ごろから自分を「女の子」と意識し、男性器を「取ることはできないか」と、心の性と体の性の不一致を保護者に訴えた。
就学を控えた昨年1月、保護者が「女児として受け入れてもらえないか」と知人に相談し、教委が受け入れを検討。一方で大阪の病院が男児を性同一性障 害と診断、担当医は「一生このままとは言い切れないが、子供が生活しやすいように対処するのが基本」と助言した。保護者はこの診断書を提出して学校、教委 と話し合い、女児としての受け入れが決まった。
教委は18日に「診断と児童への配慮を最優先した」と説明。学校の教職員は経緯を知っているが、他の児童と保護者には知らせていない。
男児は入学当初から、名簿は女児の欄に掲載され、教諭は「君」でなく「さん」で呼ぶ。身体検査やトイレも女児扱いで、昨夏の水泳授業も女児用の水着で参加。周囲の児童も女児として自然に接している。
性同一性障害に詳しい専門家の多くは「本人と家族が希望し、診断も受けているので、行政側は前向きに対応した」と評価する。半面で心配するのは、思春期に向けた今後の対応。
「体が男性化した時に周囲がどう対応するかなど、長期的な問題となる」と東京武蔵野病院の針間克己医師(精神科)。針間医師によると海外では小学生 が性同一性障害と診断される事例は多いが、第2次性徴を過ぎると肉体的な性別に戻る場合が多い。障害を診断された子供数十人のうち、大人になっても障害を 抱えていたのは1人だけという報告もある。
学校側も「小5から始まる臨海学校など宿泊を伴う場合、本人が男女の性をどう認識するか…」と不安も漏らす。教委は「専門家の指導に従って状況を見極めながら対応したい」と話している。

■各界の性同一性障害の例
カルーセル麻紀(タレント) 63歳。昭和48年にモロッコで性転換手術。特例法施行と同時に戸籍の性別変更を申し立て、本名も徹男から麻紀に。
  ★上川あや(世田谷区議) 38歳。戸籍上は男性だったが、平成10年に障害を診断された事実を公表して15年の区議選で初当選。昨年4月に戸籍の性別を変更した。
  ★安藤大将(元競艇選手) 43歳。女性競艇選手として活躍したが、13年に障害を診断。翌年から男性として選手登録。名前も千夏から大将(ひろまさ)に。昨年現役を引退した。
  ★虎井まさ衛(作家) 42歳。昭和62年に渡米して男性への性転換手術。特例法施行前から啓発活動に尽力。16年9月に戸籍の性別も変更。

 兵庫県の小学2年の男児(7)が心の性と体の性が一致しない性同一性障害と診断され、小学校が男児を入学時から女児として受け入れ、通常の学校生活を 送っていることが18日、分かった。専門家によると、第2次性徴を迎える前の小学校低学年の児童が、性同一性障害と診断されるケースはまれ。学校側はほか の児童らに知らせず「さん」付けで呼ぶなど、慎重な対応をとっている。
 この男児については、地元自治体の教育委員会幹部が会見して明らかにした。それによると、男児が自分を「女の子」と意識したのは5歳ごろから。スカートやぬいぐるみが好きだった上、「男性器を取ることができないか」などと心身の性の不一致を保護者に訴えていた。

  小学校入学を控えた昨年1月、男児の保護者が「女児として受け入れてもらえないか」と知人に相談したことがきっかけで、教育委員会が受け入れの検討を開 始。男児大阪府の病院で「性同一性障害」と診断を受けた上で「一生このままとは言い切れないが、男児が生活しやすいように対処していくのが基本」と医師 からアドバイスを受けたため、保護者と小学校、教育委員会が話し合った結果、小学校が男児を女児として受け入れることを決めた。

 男児は入学後、スカートをはいて通学。普段から女児のグループと遊んでいるほか、身体測定やトイレの際も女児と行動を共にしている。昨夏の水泳授業には 女児用の水着を着て参加。出席簿では女児の欄に名前があるのに加え、教諭も「君」ではなく「さん」付けで呼んでおり、ほかの児童も女児として自然に接して いる。小学校の教職員はこうした経緯を把握しているが、ほかの児童や保護者への報告はしていないという。

 問題は学校側の今後の対応だ。「小5から始まる臨海学校など宿泊を伴うケースで、男児が男女の性をどう認識するのか」などの声もあり、学校側は児童らへ の適切な接し方を探るため専門家から指導を受ける予定。小学校校長は「専門医と相談し、児童が普通に生活できる環境づくりに取り組んできた。今後も特異な 目で見ることがないよう、理解ある対応を要望する」と話している。

男児「女児」で通学 性同一性障害と診断 兵庫の小学校

 兵庫県内の公立小学校二年の男児(7つ)が、医師から「性同一性障害GID)」と診断され、女児として通学していることが、十八日分かった。同小を所 管する教育委員会は「入学前に保護者から相談を受けた。医師の診断の結果、女児として受け入れることが最善と判断した」と説明している。思春期を迎えてい ない子供を性同一性障害者として公的機関が受け入れているケースはこれまで明らかになっておらず、文部科学省も「聞いたことがない」としている。


 関係者の話などによると、この児童は一歳ごろからスカートやぬいぐるみが好きで、保育園に入園してからも、女児の服装を好んだという。自身の男性器についても疑問を抱く発言をしたことがあるという。
 こうした事情から、小学校入学前の昨年一月ごろ、児童の祖母から教委に相談があり、専門医に相談するよう保護者に伝えた。診断の結果、「一生涯このままとは言い切れないが、そのとき、そのときで生活しやすいようにするのが基本である」と、性同一性障害と診断された。
 この診断結果を受け、学校、保護者の三者で話し合い、女児として入学させることを決定。入学する小学校の全教職員に理解を徹底したという。
 児童は女子トイレを使い、プールの水着なども女児用を使用。身体測定も女児として受けさせているほか、出席簿も女児の名前の欄に記載し、教諭が名前を呼ぶさいには、男女とも「さん」づけにしているという。
 教委は「現在までの学校生活で、問題があったとは聞いていない。男女の体格の特徴などが顕著になる今後については、その都度、保護者とも話し合い最善の方法で対応したい」としている。

性同一性障害GID)学会理事長の大島俊之神戸学院大法科大学院教授の話  「小学校低学年がGIDと診断されたことに驚いている。幼いころに無理に体の性を強いるのは心理的負担になる。その意味では小学校の対応は基本的には良 い。だが周囲の同級生や親、先生がどう理解して対応できるか。今は低学年だから問題が起きなくても、今後成長が進むと外見の違いも目立ち、問題が生じる可 能性もある。その時、周囲は受け入れられるのか。小学校や教育委員会が今後の流れをどこまで視野に入れているのか、まだ見えてこない」

Japanese boy in elementary school as girl

TOKYO -- A young boy who believes he was born the wrong sex was allowed to enroll as a girl at an elementary school in southwestern Japan, a school official said Thursday.
The 7-year-old boy entered the school as a girl in April 2005 after he was diagnosed with gender identity disorder at age 6, a spokesman for the local school board said. The Japanese school year starts in April.
The official, who spoke on condition of anonymity to protect the identity of the boy and his school district, said the boy's name is listed with girl students, and that he attends a girls' gym class and uses the girls' bathroom. The boy, who's in the second grade, wears a girl's swimsuit at the school pool.
"At this point, we are relieved that the child was accepted into first grade and is being raised in a healthy manner," the official said.
The school is in the southwestern prefecture of Hyogo, about 270 miles west of Tokyo.
The school's decision is highly out of character for Japan's public school system, known for learning by rote and having little tolerance for children who don't fit in. Awkward children can be mercilessly bullied by other students.
The school has not told other parents about the switch, and it was unclear whether any of the students knew the boy's true gender.
The school official said there had not been any complaints from other students or from the boy's parents since his enrollment. He said the school district would watch his case closely and reassess the decision as the boy reaches puberty.
Katsuki Harima, a psychiatrist specializing in gender identity disorder at Tokyo Musashino Hospital, said the decision to allow the boy to enroll as a girl seemed appropriate, but would get complicated as he grew older.
Harima said the boy is not old enough to determine whether he really has the disorder. A boy who behaves like a girl does not necessarily have gender identity disorder and he could discover as he grows older that he wants to be male, Harima said.
"I am a bit concerned about the child's future," he added, saying he has never heard of a case like this before at an elementary school. "There will be problems."

Japan beats taboo by letting boy act as girl

A seven-year-old Japanese boy with a gender identity disorder has been given permission to attend school as a girl in another sign that the country is relaxing its traditionally rigid attitude towards sexual identity. Local media reported yesterday that the boy, who has not been named, was diagnosed with the disorder before he started primary school in April.
He is said to have complained that he felt uncomfortable being a boy and asked his parents if he could have a sex-change operation. Japan's first such procedure took place in 1998, but patients must be aged 20 or over.
The school, in Kobe, western Japan, agreed to enroll him as a girl after consulting his parents and doctors in what is thought to be the first decision of its kind in Japan. According to reports, he will be allowed to use the girls' bathrooms and changing facilities, and to wear girls PE kit. Only his teachers have been informed of his condition.
An estimated 10,000 Japanese say they have some form of gender identity disorder - a belief that they were born the wrong gender and a wish to live, socially and physically, as members of the opposite sex.
In 2004 they won a significant victory when the law was changed to allow some transsexuals to alter their name and gender on official documents. A year earlier Aya Kamikawa became the first transsexual to enter politics when she won a seat on a local assembly in Tokyo.
Last year doctors agreed to relax the conditions for sex-change operations that had required patients to gain approval from a medical ethics committee. But the country's transsexuals say they have some way to go before they are fully accepted.
A 44-year-old who had fathered two children before changing sex recently failed to register her new gender after a court said it would "confuse" her sons, aged 12 and 14. The revised law allows only transsexuals who are over 19, single and childless and have undergone a sex change to change their official gender.

School accepts lad with gender identity disorder as girl

KOBE (Kyodo) An elementary school in Hyogo Prefecture has accepted a 7-year-old boy with gender identity disorder as a girl, a local board of education said Thursday.
The second-grader was diagnosed with the disorder before entering school, a board official said.
Gender identity disorder occurs when someone's gender identity differs from his or her biological sex. The board of education and the school decided to treat the pupil as a girl after talking with the child's parents from January to March of last year, the official said.
Since entering school in April last year, the child has undergone health checks with girls and uses girls' restrooms while attending swimming lessons in a girl's swimsuit, the official said.
The pupil is listed with the girls on the class roll, the official said.
Experts said it is rare for children to be diagnosed with the disorder before they develop secondary sex characteristics and for a school to treat such kids as members of the opposite sex.
An education ministry official said there are no known cases in which a boy with the disorder has been accepted as a girl at a school.
More than 10,000 people in Japan are thought to have the disorder.
