A.少なくとも6ヶ月続く、経験した/表現した性別と、指定された性別との著明な不一致。以下の2つ以上によって表れる。
1. 経験した/表現した性別と、第一次および/または第二次性徴（または、早期青年では予想される第二次性徴）との著明な不一致。
2. 経験した/表現した性別との著明な不一致を理由とした、第一次および/または第二次性徴から解放されたいという強い欲求（または、早期青年では予想される第二次性徴の発達を阻止したい欲求）
3. 反対の性別の第一次および/または第二次性徴を獲得したいという強い欲求
4. 反対の性別（または、指定された性別とは違う何らかの代わりの性別）になりたいという強い欲求
5. 反対の性別（または、指定された性別とは違う何らかの代わりの性別）として扱われたいという強い欲求
6. 反対の性別（または、指定された性別とは違う何らかの代わりの性別）の典型的な感情や反応を持っているという強い確信
B. その状態は、臨床的に著しい苦痛または、社会的、職業的または他の重要な領域における機能の障害と関連している。

性分化疾患を伴う（例えば、先天性副腎過形成などの副腎性器症候群やアンドロゲン不応症候群）
コード番号の方法：性別違和と性分化疾患の双方のコード番号をつけよ。

性別移行後：その人は望みの性別でのフルタイムでの生活に移行し（法的な性別変更をする場合としない場合がある）、少なくともひとつは反対の性別への医学的治療、すなわち、反対の性別への定期的なホルモン治療ないし、望みの性別へと適合する手術（例えば出生時男性における陰茎切除、造腟、出生時女性における乳房切除術や陰茎形成術など）を受けている。

Gender Dysphoria (in Adolescents or Adults)
A. A marked incongruence between one’s experienced/expressed gender and assigned gender, of at least 6 months duration, as manifested by two of the following :
1. A marked incongruence between one’s experienced/expressed gender and primary and/or secondary sex characteristics (or in young adolescents, the anticipated secondary sex characteristics)
2. A strong desire to be rid of one’s primary and/or secondary sex characteristics because of a marked incongruence with one’s experienced/expressed gender (or in young adolescents, a desire to prevent the development of the anticipated secondary sex characteristics) .
3. A strong desire for the primary and/or secondary sex characteristics of the other gender
4. A strong desire to be of the other gender (or some alternative gender different from one’s assigned gender)
5. A strong desire to be treated as the other gender (or some alternative gender different from one’s assigned gender)
6. A strong conviction that one has the typical feelings and reactions of the other gender (or some alternative gender different from one’s assigned gender).
B. The condition is associated with clinically significant distress or impairment in social, occupational, or other important areas of functioning.

With a disorder of sex development (e.g. a congenital adrenogenital disorder such as 255.2 [E25.0] congenital adrenal hyperplasia or 259.50[E34.50] androgen insensitively syndorome).

Posttransition:The individual has transitioned to full-time living in the desired gender (with or without legalization of gender change) and has undergone (or is undergoing) at least one cross-sex medical procedure or treatment regimen, namely, regular cross-sex hormone treatment or gender reassignment surgery confirming the desired gender (e.g., penectomy, vaginoplasty in a natal male; mastectomy or phalloplasty in a natal female).
